Cirripectes polyzona
Cirripectes polyzona, commonly known as the Barred blenny, is a small/medium blenny. Their colour is very variable. Some are mostly cream-white, with a blotchy horizontal line and some very light brown vertical bars and net markings on their head. Others are mostly dark brown to purplish with very prononounced vertical bars, and some even have red markings on their head.
These fish inhabit shallow outer reefs where they live usually solitary in algae rich areas. In an aquarium they're reef safe and a very good algae eater. They're quite territorial though, so best place them as one of the last fishes in the tank and only in a tank big enough to form a territory and leave room for the other fishes. Don't combine them with other blennies, unless you've a very large tank with many hiding spots. They feed mainly on algae and small invertebrates, so if there is little algae in the tank high quality algae flakes or pellets should be supplemented!
